Do you administer Writing CBM's to your students either in whole group or students in the RtI process? Are you having a difficult time recording all the information gathered from doing so? If you collect Total Words Written, Words Spelled Correctly, and Correct Writing Sequences for students in grades K-6, this form will be very helpful to you. I was unable to find a method for data collection and this two page format has been extremely helpful to me.
This product does not include specific directions on how to administer the prompt. It does explain that there is one minute of "think time" after reading the prompt to your students followed by a three minute writing session. The graphs can be used by adding dots for the data points or you can teach your students to create a bar graph on them. Simply add the dates below each graph. Also, if you use a program such as Aimsweb, there is a place that you can record norm based percentiles with a t-chart beside each graph but they are not included with this product.
Also, this product has been updated to include a set of writing prompts to use for the whole school year. Simply print these prompts on card stock, cut out and laminate, and write the date you used each prompt in the given space in order to remember when you used each prompt.
