This is a one-sided document on writing the equation of a line. The student is given the choice to write their answer in slope-intercept form or point-slope form. There are 14 questions total covering 5 different types of given information: slope and y-intercept, a table of values, a graph, a point on the line and the slope of the line, and 2 points on the line. Key included. All answers work out nicely.
