You do not have a true picture of a student's problem areas by looking at the overall score, placement, and color on i-Ready. You MUST look at the individual scores on each domain to understand how to help them improve.
This is especially helpful when creating groups. The only way to see this information is to look at each separate student individually. I needed a way to see them all together in a list.
Working in EIP (Early Intervention Program) this year, I even must look at students from different classes. Once I have determined each student's greatest need area, I can then place them in groups by need.
There are two pages each of Reading and Math domains so that you can print them out two sided. I left the top area blank so that you can use this checklist at different times. Right now I am looking at the "Previous Year" report to compile my list. But, it could be used after each "Diagnostic" test. There are 15 lines for students per page.
The last 2 pages are for you to place students in groups by the domain that is the greatest need.
