This is a great tool to keep track of the reading levels of your students. It can keep you organized as well as be sent home for parents to see how their child is growing.
There's a place to write the date tested, the name of the story, whether it's fiction or nonfiction, the accuracy percentage they scored, their comprehension level, their fluency level, and whether they tested instructional, independent, or at a frustration level on the story.
The levels and grade equivalences are based off of Fountas & Pinnell, however it could be used with any running record for guided reading levels.
