This is the document that I use for my class disclosure each year. I have learned over the year that if I send something home with a signature at the very end, no one actually looks at anything. By breaking things up into the categories that I am likely to get questions on (sometimes more than once . . . or a lot more than once), to make the policies and procedures a little more clear. Also, by having both the parent or guardian sign each section, as a teacher I can refer back to that signature.
The first few pages of this document are a lightly scrubbed version of the document I use. I put prompts in () for you to add your own information. I like to write in a light and casual style so that things do not get too heavy, but also so that the important information is conveyed. You can edit the document however you wish to suit your needs.
I included a copy of my disclosure at the end of the document so that you can see what a completed disclosure document might look like.
