I have used a Multidimensional Fluency Scale meant for students as a guide to write a kid-friendly version of a fluency scale. This rubric allows students to assess their own fluency and the fluency of their peers with an easy to follow rubric.
These reading graphs can be used to show student growth at parent teacher conferences or in goal setting with students. I used the Fountas and Pinnell guidelines for instructional reading levels in each grade.
These are 6 generic reading reflection questions that can be used in a small group or whole class. I have mine in a bag and then let one student choose a question from the bag. Everyone has to answer that question before leaving.
