On this AR Reading Log there is a place for them to write the Book Title and Author's Last Name. They are to check off when they have read it 2x's and when they took a test. After they take an AR quiz, they then fill out how many points they earned, their percent, and have their parents inital their reading log.

This math activity integrates both math and art. Students create a "fraction flower," with a fraction (in simplest form) as the flower's center. On the surrounding petals, students find three different equivalent fractions and both the percentand decimal conversion of the center fraction.
