In-Class Video Permission slip

You can use this freebie to keep track of parental permission for students to watch movies in class that might be outside of their age range. The permission slip explains the academic reasoning and benefit to the movie being shown. There is space to be able to provide the movie rating and the reason. This permission slip also informs the parents that if they do not comply with their student viewing the film, the student will be provided with an alternate, but relevant assignment.
