Extra Credit Proposal

Created by
Leanne Gentry
Many students ask if they can do extra work to bring up their grade, which in turn gives teachers more work to do. This form is a way for the student to submit a proposal for them to design their own extra work. All I, as a teacher, need to do is grade it based on what they have proposed, and I have accepted. Our school has a participation grade as well as a homework, test, and quiz grade, so the form can be modified to fit your needs.

I also keep a word wall as a word bank for these quizzes. This template allows students to record their responses for a whole week on one paper. I recommend trading and grading every day, then colle
