I always keep these assignment refusal forms in an envelope on my desk and I have my highschool students fill them out if they are actively choosing to NOT do an assignment! I use this Cooking Lab Behavior and Checkout Sheet to help me keep track of how well students are doing in their cooking labs! I have every kitchen in my foods and nutrition and foods 2 class grab one of these and fill it out at the beginning of the lab. The kitchen group leaves the sheet on the counter where I can easily access it. Students start with a 100% for the lab, however, if they cuss, leave their kitchen, towel snap, goof off, etc I write that next to their name under the misbehavior
