Many students ask if they can do extra work to bring up their grade, which in turn gives teachers more work to do. This form is a way for the student to submit a proposal for them to design their own extra work. All I, as a teacher, need to do is grade it based on what they have proposed, and I have accepted. Our school has a participation grade as well as a homework, test, and quiz grade, so the form can be modified to fit your needs.

This is a great editable token system, and easy to use! Step 1: Edit the document to meet your student! (change name, and reward) Step 2: Laminate the page. Cut out yellow rectangle. Cut out token stars. Step 3: Add velcro to the back of the stars, the boxes on the chart and on spaces on the back.
