On this form: - It covers basic info about the quiz (when it was, the topic, student original score, etc) - It asks students to explain why they did not do well the first time - It holds them accountable for doing more work to improve their understanding of the material and document what they did (study using quizlet, complete missing assignments, meet with teacher or tutor, etc.) -

I suggest having your students hand-copy these rules from the Smartboard or projector. When students hand-write the rules, they learn them better, and have little grounds to say "I didn't know that I couldn't do X/Y/Z." (Have a few copies made for students with disabilities or for students who miss class on the day when these rules are shared.)
