They tear off the bottom for a reminder and submit the top to me. I staple it to the test and put it aside in a designated folder so I dont have to search for the test or quiz at the last minute.

Back to School Editable Pirate Themed Cover Sheet for Weekly Graded Papers

We send Take-Home Folders home once a week. There are always a stack of graded papers and several tests or quizzes that I prefer to know parents look at. In order to help out my busy parents, I ask them to sign just one page as opposed to several. I created this form for this reason. After filling out the form with test/quiz grades, I just staple it to the graded papers going home for that week. It's quick and simple, the parents are happy, and I'm happy that they at least know their child's tes

Assignment Re-Test Form

This Assignment Re-Test Form provides a structured system to help students reflect before retaking a test or quiz. In my classroom, I found that students often perform worse on retakes when they do not take time to identify what went wrong. This form slows them down and requires accountability before a second attempt. Students identify their original score, reflect on possible reasons for their performance, and choose a next step. This process encourages responsibility, improves preparation, and

The document is printable and editable in MS Word. I usually print on colored paper, cut them out, and give one to each student per nine weeks, or two per 9 weeks (depending on the school grading period). This DOES NOT let students get out of an assignment, but allows them time to turn it in. Students must keep up with t
