Many of us are using the NWEA MAPS assessment to monitor our students' progress in both reading and math. This is a worksheet that I created that enables students to take ownership over their MAPS scores, and encourage them to make growth. I used two questions on my form:
What subject are we doing? (I did multiple choice answers)
What is your name? (they fill in their own name)
Students fill out the questions before leaving for the restroom. The Google Form will populate into a Google Sheet with the time stamp of the day and time a student left the room to use the restroom.

Filming Code of Conduct
This filming code of conduct I give to my students on their first lesson for the year. We read it together and then the students sign it. This is then kept in the front of their Media file to refer back to if/when they have questions or are breaking a rule. A copy of this is also emailed home to all parents.

Reading Challenge Tracker
I created this Reading Challenge Tracker to keep track of my student's books and amount of hours that they read at home with their parents. Their parents had access to edit the Excel Spreadsheet and write in which books they read at home or they could message me which books and I would put it in for them. After they write in the books, I would go check and highlight with a color. The hours of reading at home mattered more than the amount of books, because the longer they are reading, the better

Participation Challenge
I use this in my 7th grade ELA class to challenge students to participate. This encourages them to be repsectful to other students talking, raising their hands and not shouting out the answers, and to particpate more in the classroom. When they do the following, they will earn a sticker. At the end of the week, if they have all 5 stickers they will choose from the prize box.
