I have my students fill this form out when I have given them an intervention sheet they need to complete and have signed by a guardian. When they return the intervention form, I check them off and they are not able to retake a quiz or test. The intervention sheet will have QR Codes that will take them to activities to studies.

This is a reward for students to raise their grades. I give students a choice between eating with the principal and the counselor. If the student reaches a predetermined goal (i.e. increase grade from 54% to 64%) then they are rewarded. I kept the file as a word document so you can make modifications.
