I 3-hole punch these and use them in folders with prongs in the middle. It helps keep constant contact with parents, as well as help the students keep organized and take home any information. I usually label each pocket with labels from the Dollar Tree that say “Keep at Home” and “Return to School." I have found great success with these, and it usually keeps the families and I on the same page working together.

End of Year Parent Survey
Good teachers are always looking for opportunities to reflect and improve, and parent feedback is a huge help! I send these home during the last couple of weeks of the school year, and they help me get a feel for what things I might want to keep or change for the following year.
If you would prefer not to include a line for the student's name at the bottom, simply trim off the bottom part before copying; however, leaving it as an option always works for me!
