Behavior Reflection Sheet
We use this in our first grade class to help students understand the consequences of their actions. They "Stop and Reflect" in order to make a better choice the next time.
*I have recently updated the document to include a middle grade version for 3-5 with more grade appropriate wording and formatting!
After it is completed, fold in half to keep private. The student signs it, the teacher signs it, then it goes home for the parent to sign.

Parent Teacher Conference Form - Strengths / Concerns EDITABLE
I couldn't find a parent/teacher conference form that I liked, so I made my own. I have included general strengths/concerns and academic strengths/concerns. I have found that this form is a good starting point for my parent/teacher conferences. This form is a Word Document and EDITABLE. This form is intended for my third grade students.

Behavior Consequence Form (Discipline)

I created this "behavior consequence form" a few years ago when I realized that a detention doesn't help a child learn after they demonstrate inappropriate behavior. This file has no preview.
