I made these username and password cards so my students could independently use the technology lab. I plan to fill in student information, cut them out, and laminate them. They will then be made available to students when they are ready to use the classroom, media center, or technology lab computers. They could also be sent home so students could assess internet resources after school.

Ask parents to review the instructions and sign the slip after reviewing the assessment and making corrections with their child. The form aligns to and includes the Common Core State Standards (page 1: for grades 1-2; page 2: for grades 3-6) to better inform parents.
