Test/Quiz Reflection Form
This is a form designed to encourage students to mindfully reflect on their test/quiz scores. I give these to my students printed on a colored paper before they recover their test/quiz grade. It allows them to reflect and then I can use it as the cover page for their test/recovery test when scores go home! Great way to encourage parents to discuss with their students about their assessments. EDITABLE so you can change to fit any type of test!

Helps keep home in the loop about what is going on with their students Reading Comprehension according to the books they've been taking quizzes on. This quick, simple note helps head-off common questions about how it works in as a grade and opens door for communication about the scores and progress.

Total Points System - Grading Poster
The total points grading system is a more authentic and fair way to grade students in almost any situation. Categories of assignments (tests, quizzes, homework, etc.) are pre-weighted, allowing more flexibility with assignment distribution.
This document, which can be blown-up to poster size, helps students and parents better understand how a "total-points" grading system works and how they can keep track of the current grade.

This document is intended to be a base format for administrators and supervisors to use in collecting data. It is recommended that questions be theme based for ease of categorizing responses. Adding questions is possible but not suggested as busy teachers traditionally resent required paperwork to begin with. Quick and simple will result in higher number of responses. Handing out one survey per month encourages feedback without increasing the work burden.

Accelerated Reader Nonfiction Parent Sheet
This is a resource I send home if a child does not pass an Accelerated Reader quiz. Parents are asked to help the child by asking some comprehension questions, then signing and returning the paper when they feel their child is prepared and ready to take a new quiz. This resource is for nonfiction texts.
