This is a simple 15 question habitats quiz. It has true/false questions and multiple choice. I asked questions about different animals, plants, and the weather of different habitats such as rain forest, desert, arctic, and ocean.

Students have a short multiple choice portion and afterwards have to write living or nonliving on the line.

This technology survey was sent home to see what students had access to technology at home. I work in a very low income area so it was important for me to see who needed the priority in the classroom in terms of online usage. It is editable and in both English and Spanish. I also sent this paper home after students completed diagnostic tests in I-Ready (Florida).
