I created this years ago but redid some of it this year. I have the students evaluate themselves before conferences. Then I let the parents see how the student sees how they are doing. You should be able to edit this to fit your needs.
This is a review (could be a quiz) that ask the students to identify and name the lines, shapes, angles, or transformations.

Go over what a goal is. Discuss how you can create goals that are too easy or too hard. Then give examples on what a school goal is along with steps to attain it, along with personal goals. Students will fill this out after the class discussion. It is great to see what goals they set for themselves and how you can help them reach their goals.
