This is a narrative report that I compiled after I tested a 5th grader in a local school. Writing a QRI report is tedious and difficult. I included this report to give you an example of the format you can follow when writing a report such as this, as well as getting some ideas about things to focus on when testing students. This plan was written to accommodate the 3 tiers of the positive behavior support system. It allows for a lot of room when it comes to individualizing management for each child. This plan was written to send home as an informative tool for parents: it contains a description of the classroom atmosphere, norms/procedures, and also some basic information about ways in which children follow the rules best. I used this plan in my classroom and with my parents last year and it worked like a dream!
