Includes all the pertinent contact information, as well as insights as to the child's likes, dislikes, attitude towards school, etc. I have used this form for years and absolutely love it! The form is formated for the 2013-2014 school year.

This is a great printable to print in bulk at the beginning of the school year. It has separate categories to record deadlines, things to remember, and things to follow up on. Stick them in a 3-ring binder and you'll be good to go :)
