This is a great pdf that I created to help with bathroom and toilet training documentation in my self-contained classroom for students with special needs. This can be used with any age student (I personally have used this with kindergarten through high school aged-- older students typically are in adult briefs at that point) This pdf is very simple to use-- there is space for 4 students per page. My own education history
I attended James Madison University in Harrisonburg, Virginia. I graduated with a B.S. in Interdisciplinary Liberal Studies in 2006. I continued my graduate studies at James Madison University, and graduated with a M.Ed. in Special Education in 2007.

I currently teach in a Transition from School to Work (TSW) program at a public high school in the Phoenix-metro area.
