I made this log in order to document specific behaviors. I made copies and carried it on a clipboard to help all staff involved jot down quick notes. This was very handy when it was time to present information on specific behaviors (what were the behaviors, frequency, etc.).
This is a simple form that you can fill out either before or during the conference. You can highlight student's strengths, areas of growth, and concerns. Also, there is a section for comments and/or questions.
You can give this filled out form to the parent so that they know in what areas they can help their child.
I made these to help keep the teacher's desk in my classroom neat and organized. You can print these on card stock, laminate, and cut them out. I paper clip these to any area or document that needs it but you can use these however you'd like.
These are perfect for teachers that job share and need to keep things neat, organized, and clearly labeled for each other to find.
