This form can be utilized to gather in-depth information from a grade level teacher about a student before the ARD or IEP meeting takes place. By obtaining the information, the special education teacher can: * create academic goals and objectives * create or revise a behavior intervention plan (BIP) if needed * be more knowledgeable in denoting progress on current goals and objectives * have documentation on modifications and/or accommodations the student is using in the general education class
