Quick and easy IEP snapshot. Make a copy of the document for each student on your caseload to share with staff at your school to share vital IEP information with general education staff. The document lives in Google Docs so you can edit and change things as you see fit. The whole document is completely editable. Disability categories are based of my state's resource categories but the document is fully editable so you can change categories to match your state or needs.

At my school each child receives a composition notebook as a form of daily communication, I find that my students are old enough to write their own response but young enough that a template really speeds along the process. This log allows for open communication between the parent and yourself. It also allows students to really think about their day and be responsible for their actions.
