This product contains one school counseling intervention planner and documentation form, one counseling roster form, and one session tracker form. I like to keep these forms handy on my desk every day. I use the Intervention Planner to develop a plan of action to provide counseling interventions to students who are referred to me, and I document the students I see using the Counseling Roster and Session Tracker.
