This is a resource I use daily in my classroom. You can use is for your ESE and regular ed children. I like to attach it to any work that the students received assistance on. It is great documentation to keep on student work.
These activities were created for the book Polar Bears by Mark Newman from the third grade Making Meaning kit. We started with the double entry journal. Students would write their wondering about the book in the left column. As we read through the book they would write what they learned in the left column. Also included is an exit ticket and a 5 question assessment that you could use for a grade.
Twenty two weeks of spelling words to add to your third grade classroom. There are three word lists on each page. Every Monday I would staple the spelling list into each child's agenda/planner. This was a great way to make sure they didn't lose their list. On Fridays I would give them the spelling test. Enjoy :)
Reading Logs are a great way for students to track their reading throughout the year. You can take them as a grade or use as an incentive. My students were able to turn in a completed sheet for a treat. The file is as a word document so you can change the information in the header!

The text feature journal entry is used during independent reading time. Each child writes an entry in their journal about a nonfiction book. They are to touch on each bullet point from the worksheet.
