This form communicates bi-weekly (or weekly, or monthly) behavior, assignment completion, and any other necessary comments to parents. I have the students self-evaluate themselves first, and then I revise if necessary in a different color pen. In my classroom, parents are required to return this form signed. Many choose to respond with comments. It is very helpful, so that there are no major surprises come report card time. Dyan Branstetter
