These math conference forms for first-grade teachers will help you to keep track of your students' progress and your teaching. In addition to the original individual student conference forms, I've recently revised this resource to add small group forms and class status forms.

There are multiple pages for use throughout the year during Math Workshop conferences. Each page has the first-grade Common Core listed for easy reference as you confer with your students or small groups.

On the original individual form, conferring notes for several conferences may be noted on the same page to assist you in meeting individual learning needs as you build on previous teaching points.

The small group forms allow for strategic lesson planning and anecdotal record keeping. Common Core State Standards are noted on each form.

Finally, use the class status forms to keep track of your class' progress toward mastery of the standards.

All of the forms will help you stay organized and prepared for parent conferences and report cards.
