This is a form that I made to use at parent teacher conferences.
The form allows you to record things you may want to discuss during the conference so that you are prepared and not shuffling through all your records for those important scores.
There are spaces for Benchmark Assessments in Math, Reading, and Writing as well as Guided Reading Levels throughout the year.

This is a template that I use for planning my units that I am going to teach. It allows you to plan backwards and have evidences to show that your students are achieving the skills you are teaching them. The template is set up to plan your unit in 3 steps: Standards and Goals, then Assessments (both formative and summative), then planning a step-by-step (lesson-by-lesson) schedule for the unit.

This is a mat that students can use to count to 100. On the 100th day of school students can make a necklace with Cheerios or fruit loops, but first they should place the fruit loops on the ten-frame mat until they reach 100. Then they can remove them and string them onto a piece of twine or yarn to make a necklace. Once the necklace is complete they can wear it all day and count the fruit loops as they eat them. Counting to 100 3 times in one activity.
This is a book that I created to accurately assess the kindergarten skills within the common core standards. I provide at least 3 different ways to asses each standard. It is set up so that you can print 1 page per standard. On each page is 3-4 different ways to asses that standard. So you can print that 1 page 20 times (however many students you have) and have all the assessments you will need for that standard. At the end I list all the common core math standards and best practices.

Students record their observations after planting a lima bean in soil or in a plastic bag with a wet paper towel. Each day or weekly the students observe their bean and record their observations on the chart. Helps with making predictions and observing experiments.

This is a journal that I used in 3rd Grade and 5th Grade Science class. In 3rd grade were studying the patterns in the sky. They were asked to observe the sky each night for two weeks and draw a picture of the sky & moon. Then they were to write about what they observed

This is an excel file that displays the Common Core Standards for Kindergarten Literacy. It gives you three times to assess then a progress report slot. Then it gives three more times then a report card slot. Makes filing out report cards so much easier.

I made theses 100 charts that help students learn to count by 2s, 3s, 4s, 5s, 6s, 7s, 8s, 9s, and 10s. Each chart is highlighted based on the numbers that you are counting by.

Monitor Student's classwork and record whether they are on task or are given extra time to do their work. As a teacher I have found it very important to record everything. Have the proof to back up your teaching as well as student's progress.
