These student information cards are so handy when I need to contact a parent. All the information I need is right there... don't need to look it up in the computer or flip through my student files. I copy these cards on card stock, have the parents fill it out, laminate, punch a hole in the corner, and then put on a ring. I keep it on my filing cabinet for quick and easy access.

This is a one page sheet used to gather basic information from the parents and guardians (names, phone numbers, address, email address). The form also asks if parents would like to receive information via email or paper newsletter. If purchased, you will receive the document in Word so that you may edit it to fit your preferences.
