This activity aligns to 5th grade Common Core Standards 5.G.3 and 5.G.4, requiring students to categorize shapes into a hierarchy, based upon their properties or attributes.
There are two versions included for easy differentiation. Version 1 includes a word bank for additional student assistance, perfect for Special Education students. Version 2 offers no addition help, perfect for higher or more advanced students.
Answer keys for both versions are included!
This is a performance assessment quiz on coordinate graphing. This resources contains 3 different Coordinate Grid Graphing Quizzes, each a different level of difficulty. In addition, each quiz contains a coordinating answer key.
Version 1: Requires identifying and graphing in all 4 quadrants of the coordinate system.
Version 2: Requires identifying all 4 quadrants of the coordinate system, but graphing only in quadrant 1.
Version 3: Requires identifying and graphing only in quadrant 1. (On
This activity was used in teaching the new Common Core State Standards for 4th grade Math:
4.NF.5. Express a fraction with denominator 10 as an equivalent fraction with denominator 100, and use this technique to add two fractions with respective denominators 10 and 100.2 For example, express 3/10 as 30/100, and add 3/10 + 4/100 = 34/100.
4.NF.6. Use decimal notation for fractions with denominators 10 or 100. For example, rewrite 0.62 as 62/100; describe a length as 0.62 meters; locate 0.62 on a
This modified handout could be used as part of the Practical Living Curriculum, Vocational Studies, or as part of transitioning for Special Education students.
After searching (and even finding and using for 2 years) some great standards based assessments for student Math Data Binders, I still didn't have exactly what I needed or wanted. So, I decided to create my own!

This handout was used in 5th grade Math while teaching the Common Core State Standards. Students are required to draw base-10 blocks and write numbers in Base-10 Numerals (Standard Form), Number Name (Word Form) and Expanded Form using Multiplication.
This activity was used in teaching the new Common Core State Standards for 5th grade Math. Students roll 3 die. After rolling, each number is used as a dimension on the rectangular prism and is recorded on the diagram, in any order, length, width or height. Students then input the dimensions in the formula for volume, either V=lxwxh or V=Bh and puts the final answer in the last column.

This was used in 5th grade Math as I was teaching the new Common Core State Standards. This activity requires students to write a given decimal, to the thousandths place in base-10 numeral, number name and expanded form.
This activity was used in teaching the new Common Core State Standards for 4th grade Math:
4.NF.7. Compare two decimals to hundredths by reasoning about their size. Recognize that comparisons are valid only when the two decimals refer to the same whole. Record the results of comparisons with the symbols >, =, or <, and justify the conclusions, e.g., by using a visual model.
Students roll 2 die, recording the number created in the first two boxes. Students roll again, recording the number
This activity can be used to teach or review are and perimeter. Students pick dominos out of bags, label the diagram, draw a picture representation, and use the number of pips in each section as a dimension of length or width to compute the area and perimeter.
This activity was used in teaching the new Common Core State Standards for 4th grade Math:
4.NF.4. Apply and extend previous understandings of multiplication to multiply a fraction by a whole number.
Students draw 2 dominoes from a bag, recording one vertically as the fraction and the other horizontally as the whole number. From here, they compute the product following the appropriate steps (converting the whole number into an improper fraction and multiplying straight across).
This activity was used when I taught 5th grade Math with the new Common Core State Standards. Students are given a picture of a domino and have to give the fraction form, as well as mixed or whole number form.
This is a chapter graphic organizer, broken down according to lessons, for Kentucky Harcourt Science textbooks for 4th grade. This makes a great activity for students to ensure students are reading lessons, but also getting the main points and important aspects of each lesson.
