Formative assessment has never been easier! This pack has 5 sets of printable 4th grade math exit slips or exit tickets for each of the 4th Grade Common Core Math Standards. As a BONUS it also has a digital set of exit tickets included!

This pack was recently updated to include both digital exit tickets for Google classroom AND printable exit tickets.

These exit tickets have been revised and updated for ease of use!

  • There is a student self-assessment at the bottom of each exit ticket
  • They have a "cut and go" design
  • The design utilizes space and has 4 tickets per page

The key features of this set of 4th Grade Math Exit Slips are:

  • They include both computation/operation skills and problem solving
  • There are 5 slips for each math standard,
  • Each exit ticket is labeled with the Common Core Standard for easier organization
  • They provide a quick and easy check of student understanding
