Ensure that your students master the Common Core Math standards!

This is a set of assessment checklists.

Part 1: Each page has one 5th grade Math Common Core State Standard listed, with room for 30 students per page, to document standard mastery. There is room to note beside each student if and when they are below expectations, at expectations, or above expectations for that standard. Simply write in the student names and the dates they were assessed in the proper column based on their assessed mastery.

This is a quick and easy way to keep track of each CCSS Math standard and how the class is progressing in mastering it. A quick view makes it easy to see if a standard needs to be retaught to the whole class, a small group, or specific students.

Part 2: Individual student pages are at the end of this file. Print one set for each student and place them in a binder, documenting that student's overall progress in Math for the year. Each standard sub-category is broken into separate pages for clarity. Lots of room for notes as well!
