This chart is great to keep track of student reading data throughout the school year! To utilize this tool, I break down assessments or assignments by TEKS and record the student's achievement with each skill. Then, I know which students need further instruction in small group or a conference in the future.

This tool is great information to have when meeting for parent-teacher conferences, RtI, or creating your small groups.

I just make a sheet for each student at the beginning of each school year, then three-hole punch them and create a data binder with these inside.
