This Progress Monitoring / Intervention Kit

This bundle works best in a small group with a teacher.

It is designed to provide students with extra practice with Standard 6.NS.4 

Each lesson includes an Exit Slip to be used after instruction to determine the student’s understanding. Exit Slips can also be used to retest after interventions.

The worksheets are intended to be used in small groups and not meant to be used for full class lessons – although they could be if that is your preference. 

Each skill comes with  four resources: 

  • Exit Ticket
  • Tracking Sheet
  • Skills Worksheet - Guided Practice 
  • Extra Practice Worksheet - Independent or Partner Practice
  • Instructional Sheets
