A-B-C Data Collection Form

• Antecedent (A): The event or environment immediately preceding the behavior (e.g., being asked to do math, hunger, peer interaction).
• Behavior (B): The observable, measurable action the student performs.
• Consequence (C): The immediate result following the behavior that affects its likelihood of recurring.

The data, often collected by teachers or paraprofessionals, should be objective and focus on observable actions. The ultimate goal is to reduce, eliminate, or replace disruptive behaviors with positive alternatives.
