This electronic version allows educators to input events before a behavior of interest (antecedents), behaviors observed, and events occurring immediately after the behavior (consequences). ABC data collected can be used to assist in data collection procedures to determine the reason (function) of the behaviors of interest. The form has an area where the behavioral definitions are written; these can be removed or updated to be specific for the student.
