This product contains two different ABC data collection forms. I also included examples for you to see a way they can be used. ABC stands for Antecedent, Behavior, Consequence.It is a popular method for collecting data when forming Behavior Intervention Plans (BIPs) and Functional Behavior Assessments (FBAs);: however, this form is helpful in many other ways. It is a great way to see what is causing the behavior (what happens right before- the antecedent) and the function of the behavior (what the student gets out of the behavior- the consequence). I suggest using these forms to help track problem behavior and create a plan to replace problem behavior with expected behavior.
