Over the 13 years I taught and coordinated AVID, I created the following forms for interviewing potential students as well as returning students. I have added slips to give to students to remind them to come to their interview and I have included a rubric for selection. You and your site team can decide the range of points you would like to use to guide you toward the students you would like to add to the program.
