Description
In the case that a student did not make a successful score on their AR quiz, I wanted them to "think" about what could have caused this. I wanted this to be a time of reflection for my students allowing them a chance to self discover ideas or strategies to help them with their next book. After students had a chance to write down their thoughts, we would conference about their ideas and it would allow me a chance to reinforce any ideas they may have and to then intervene or point them in the right direction.
This form could even be good documentation about the students reading and their thoughts about how well they read their book.
** This sheet can be copied and cut in half. I always had a small stack of them ready to give a student during our conference after their quiz.
