Included in this pack is a student AR folder cover (six options), student tracking forms, username and password slips, reading wishlist sheet, a thoughts and ideas sheet for the student to stick their sticky notes while reading, and a teacher's AR form for recording student goals and reading ranges.

My students have a copy of each of these forms in their AR folders to keep them accountable. I use their wishlists to help stock my class library, and they use their "thoughts and ideas" page to record any thinking or questions that they may have while reading. I keep the teacher's AR form in my Student Data binder for quick reference.
