My third graders have struggled with completing assignments independently. This has worked wonders for us! They need to fill this out on their own, then as a teacher you can sign it to agree or recircle in pen what you think is accurate. Parents will have to sign if they scored a 1 or 2. This allows students to reflect on their ability to complete tasks.
