I created this data form for teachers needing a place to enter data about the number of times a student gets aggressive. You can input the number of times a child gets aggressive throughout any hour of the day. It calculates the formulas by total number of times a week, total number times a day, and total number of times an hour. This will help evaluate what time of day or day of the week the student is most aggressive. It has saved my life when taking data, and allows me to easily provide data for behavior specialists and if needed parents.
