A 4 page template that has areas for Math, Writing and Reading, with additional areas for small group instruction and workstation/centers as well. In addition to all that, there is a Guided Reading area with enough room for 3 groups to meet daily, a materials section and a whole page dedicated to teacher information, like objectives, notes, theme and spelling/ high frequency/ vocabulary words. The font I used for this template is called Pea Mique Bold and you can get it from Kevin and Amanda free pea fonts.
