This 5 problem, quick assessment helps you to quickly determine students' understanding of simplifying complex numbers. 

This is a google form, so it can be customized to fit your classroom. 

It includes the ability for the students to upload their work.  If you want to make that optional, you can click the toggle to not required, and if you want to not have that as a part of the assessment, you can easily remove it by clicking on the trash can within the problem.

This google form is great for use as a ticket out the door (exit ticket), a formative assessment, a homework check/homework assignment, or a warm up.
