Description

This five problem assessment helps you to quickly determine students' understanding of solving quadratics using the quadratic formula (including complex solutions).

This Google Form is great for use as a ticket out the door (exit ticket), a formative assessment, a homework check, a shorter homework assignment, or a warm up.

I've already customized its settings based on the most commonly used ones by my  collaborative teachers and myself. 

Since this is a Google Form, though, the great thing is that it can be further customized to fit your classroom needs!

Some of the options available to you to customize the assignment include:

  • Allowing students to upload their work (can be required for students or optional if they choose to do so--that's up to you!)
  • Scrambling the order of the problems and/or the answer choices
  • Changing the format from multiple choice/matching to constructed responses
  • Adding, editing, or deleting problems and answers
  • Requiring students to sign in or not
  • Allowing students to see their grade immediately or when you choose
  • Changing the assigned point values
  • And many more options built into Google Forms' functionality
