    • Space for thesis statement, if you require
    • Properly formatted and placed MLA citation
    • 3 paragraphs showing what to include in each (summary, analysis, and reflection)

This sample was designed for high school English classes, but can be edited for other grades or content areas. This has been extremely helpful to show my students what their annotated bibliography is supposed to look like and eliminates a lot of confusion and questions.
